Health & Safety Waiver and Participant Declaration
1. Introduction
Wushu taolu is a non-combat martial art, but like all athletic activities, it carries inherent risks. This Health & Safety Waiver ("Waiver") outlines the responsibilities and acknowledgements required from all participants, and from parents/guardians or authorised representatives submitting information on behalf of a participant.
By registering for or taking part in a competition, you confirm that you understand and agree to this Waiver.
2. Who must complete this Waiver
This Waiver must be completed by:
- Adult participants (18+).
- Parents or legal guardians completing registration for participants under 18.
- Coaches, team leaders, or authorised representatives entering participants on their behalf.
Anyone submitting information for another person must have full and lawful authority to do so.
3. Acknowledgement of risks
You acknowledge and understand that participation in wushu taolu may involve risks including, but not limited to:
- Physical exertion and fatigue.
- Slips, trips, and falls during movement sequences.
- Injuries arising from jumps, balances, acrobatics, or rapid directional changes.
- Use of traditional wushu apparatus (e.g., staff, spear, sword, broadsword).
- Environmental factors such as floor conditions, lighting, or venue layout.
- Aggravation of existing injuries or medical conditions.
You accept these risks voluntarily and confirm that participation is undertaken at your own risk (or at the risk of the participant you represent).
4. Health and medical fitness
You confirm that:
- You (or the participant you represent) are physically fit and medically able to participate safely in wushu taolu.
- You have undertaken any necessary medical checks, assessments, or tests appropriate for martial arts participation.
- You have disclosed any relevant medical conditions to the organiser where required for safety.
- You will not participate if you are unwell, injured, or otherwise unable to take part safely.
For participants under 18, the parent/guardian is responsible for ensuring the child is medically fit to participate.
5. Responsibility for equipment, clothing, and apparatus
You acknowledge and agree that:
- Any equipment, clothing, footwear, or wushu apparatus (including but not limited to staff, spear, sword, broadsword, or other implements) that you bring to the competition venue is your sole responsibility.
- You confirm that all such items are fit for purpose, in good condition, and safe for use in a competitive environment.
- You accept full responsibility for any injury, damage, or loss caused to yourself, other participants, officials, spectators, or property arising from:
- The use or misuse of your equipment or apparatus.
- The physical failure, breakage, or malfunction of any item you bring.
- Inadequate maintenance or inspection of your equipment.
- You agree to comply with any equipment checks or safety inspections required by the organiser.
Organisers reserve the right to prohibit the use of any equipment deemed unsafe.
6. Compliance with rules and instructions
You agree that you (or the participant you represent) will:
- Follow all competition rules, safety guidelines, and instructions from officials.
- Use equipment and apparatus safely and appropriately.
- Behave in a manner that does not endanger yourself or others.
- Report any hazards, injuries, or unsafe conditions to organisers immediately.
Failure to comply may result in removal from the event.
7. Photography, video recording, and media use
You acknowledge and agree that:
- Competitions may be photographed, filmed, or livestreamed by organisers, accredited media, or authorised personnel.
- Images and footage may include participants performing routines, receiving awards, or being present in the venue.
- Such media may be used for event promotion, livestreaming or broadcast, social media posts, training, judging, or educational purposes, and archival or historical records.
7.1 Consent for adults
If you are an adult participant, you consent to the capture and use of your image and performance as described above. You may withdraw this consent at any time via your profile settings or by contacting the organiser, though this will not affect already-published material.
7.2 Consent for minors
If the participant is under 18:
- A parent or legal guardian must provide consent for photography and media use.
- If consent is withheld, organisers will make reasonable efforts to accommodate this, but cannot guarantee complete avoidance of incidental capture in group or background footage.
7.3 Third-party recording
You acknowledge that spectators may take photographs or videos unless prohibited by the organiser. Organisers are not responsible for media captured by unaffiliated third parties.
8. Limitation of liability
To the fullest extent permitted by law:
- The organiser is not liable for injuries, losses, or damages arising from participation, except where caused by the organiser's negligence or breach of statutory duty.
- You participate at your own risk and are responsible for your own safety and wellbeing.
- You are responsible for ensuring appropriate insurance coverage if required.
This Waiver does not exclude liability for death or personal injury caused by negligence, fraud, or any other liability that cannot be excluded under UK law.
9. Participants under 18
Where the participant is under 18:
- A parent or legal guardian must complete this Waiver on their behalf.
- The parent/guardian accepts full responsibility for the minor's participation.
- The parent/guardian confirms that the minor is medically fit and has been properly prepared for the event.
Coaches or team leaders entering minors must have explicit permission from the parent/guardian.
10. Consent to medical treatment
In the event of injury or medical emergency, you consent (or consent on behalf of the participant) to:
- Basic first aid.
- Emergency medical treatment.
- Transportation to a medical facility if required.
Organisers will make reasonable efforts to contact a parent/guardian for minors.
11. Declaration
By completing registration or participating in the competition, you declare that:
- You have read and understood this Health & Safety Waiver.
- You accept the risks associated with wushu taolu participation.
- You confirm that you (or the participant you represent) are medically fit to take part.
- You understand your responsibilities regarding equipment, apparatus, and safety.
- You consent to photography and media use as described (or provide consent on behalf of a minor).
- You have the authority to provide consent where acting on behalf of another person.
